Origin
The serratus posterior superior originates from the spinous processes of the C7 to T3 vertebrae, including the nuchal ligament (which spans across the cervical spine).
Insertion
The muscle inserts on the superior borders of the second to fifth ribs, near their angles.
Innervation
The serratus posterior superior is innervated by the intercostal nerves arising from the T1-T4 spinal segments. These nerves are part of the ventral rami of the thoracic spinal nerves and provide motor innervation to the thoracic wall muscles.
Blood Supply
The blood supply to the serratus posterior superior comes from the posterior intercostal arteries, which are branches of the thoracic aorta.
The serratus posterior superior is a muscle located in the upper back, deep to the trapezius and rhomboid muscles, positioned between the spinous processes of the upper thoracic vertebrae (C7 to T3) and the upper ribs (ribs 2-5). It is part of the intermediate layer of the extrinsic muscles of the back and plays a vital role in respiration by assisting in the elevation of the upper ribs during inhalation. Additionally, it contributes to the stabilization of the thoracic spine, helping to maintain proper posture and spinal alignment. The muscle’s primary function is to expand the thoracic cavity, facilitating lung expansion, and to assist with posture during movements like bending and twisting.
Clinical Significance
Respiratory Function
The serratus posterior superior elevates the upper ribs (ribs 2-5) during inhalation, expanding the thoracic cavity and increasing lung volume.
Stabilization of the Thoracic Spine
The serratus posterior superior stabilizes the upper thoracic spine and rib cage during activities like bending, twisting, and maintaining posture.
