Origin
Insertion
Spinous processes of vertebrae, spanning 3–6 levels above the origin.
Innervation
Posterior rami of spinal nerves.
Blood supply
Vertebral, occipital, intercostal, lumbar, and lateral sacral arteries.
The multifidus muscle spans the entire length of the vertebral column, from the sacrum to the axis (C2). It is thickest in the lumbar region and consists of short, triangular fascicles bridging three to six vertebral levels. These muscles fill the groove on either side of the vertebrae’s spinous processes and are essential for spinal stabilization.
The multifidus muscles form part of the Inner Unit of the core, working synergistically with the transversus abdominis and pelvic floor muscles to stabilize the lumbar spine. This muscle group is vital for both static and dynamic spinal stability and is heavily involved in maintaining proper posture and supporting the spine during movements like walking or sitting.
Despite its thin structure, the multifidus is indispensable for spinal health. Dysfunction in this muscle group is strongly linked to lower back pain and postural instability. When the inner unit core muscles fail to work in harmony, the multifidus often compensates by overworking to stabilize the spine, which can lead to fatigue and strain during activities.
Bilateral contraction: Extends the vertebral column.
Unilateral contraction: Ipsilateral lateral flexion and contralateral rotation of the spine.
Proprioception: Provides feedback and fine-tunes postural adjustments.
Dynamic spinal stability: Proper activation optimizes vertebral efficiency, reducing the risk of degeneration in joint structures caused by friction.
