The posterior abdominal wall is a muscular and fascial structure forming the back boundary of the abdominal cavity. It provides support for the spine, enhances core stability, and plays a crucial role in trunk and lower limb movements.
The posterior abdominal wall is primarily composed of the Quadratus Lumborum, Iliacus, Psoas Major, Psoas Minor, and Diaphragm (posterior fibers).
The Quadratus Lumborum is a deep muscle on either side of the lumbar spine. It is responsible for lateral flexion of the trunk, spinal stabilization, and pelvic control, supporting postural alignment and aiding in movement during walking and bending.
The Iliacus, part of the Iliopsoas complex, is a primary hip flexor that stabilizes the pelvis and lower back, originating from the iliac fossa and joining the Psoas Major.
The Psoas Major is a thick muscle extending from the lumbar vertebrae to the femur, playing a critical role in hip flexion, spinal stability, and coordinated lower limb movement.
The Psoas Minor is a small, thin muscle that runs along the anterior surface of the Psoas Major. It assists in lumbar flexion and supports slight pelvic tilting, present in some individuals.
The posterior fibers of the diaphragm attach to the lumbar vertebrae. It aids in respiration and contributes to core stability by regulating intra-abdominal pressure.
The posterior abdominal wall muscles provide postural support, assist in trunk and hip movement, enhance core stability, regulate respiration, and support pelvic and spinal alignment.
The thoracolumbar fascia is a fascial structure with anterior, middle, and posterior layers. It surrounds and supports several muscles, including the erector spinae, transversospinales, quadratus lumborum, and psoas major.
The intrinsic deep back muscles (erector spinae and transversospinales) are enclosed between the posterior and middle layers, the quadratus lumborum is positioned between the middle and anterior layers, and the psoas major is enclosed within its own fascia.
The psoas fascia is a specialized fascial sheath that envelops the psoas major muscle. It connects medially to the lumbar vertebrae and laterally to the thoracolumbar fascia, helping maintain the integrity of the retroperitoneal space.
